Wayne Community Health Center is searching for a (LCSW) Licensed Clinical Social Worker or an (LPC) Licensed Professional Counselor to provide counseling for our patients and backup for our medical staff in behavioral health treatment. We would like to have someone one day per week in the clinic setting. The time is somewhat flexible and we are looking for someone who relates to all ages.

CQI COORDINATOR

Full time position responsible for extracting and analyzing data from a variety of sources, such as: databases, clinic patient charts, hospital patient charts, survey instruments and admission logs. Will work directly for the Executive Director and with the CQI Committee to prioritize, select and design CQI data collection projects. Will use statistical software and spreadsheets to analyze data and display in graphical form.
Will also serve as the Company’s Safety and Compliance Officer.

A Bachelors Degree in Health Sciences is preferred. Must have a working knowledge of statistics and data analysis, along with demonstrated skills with Microsoft Access, Word and Excel. Must be a team player and have the ability to work under deadline pressure.

MENTAL HEALTH THERAPIST

Part-time Mental Health Therapist position available. This position is responsible for the coordination of service delivery to clients with mental health concerns and in providing the following core services: diagnostic and assessment, counseling, psychotherapy and community support. Spanish and English is highly preferable.

Requires a degree in Social Work, Psychology, Counseling or related field and a Social Work/Counseling license, or be license eligible in the State of Utah. Masters or PhD preferred. Must have 5 years of relevant work experience, 2 of which should be clinical experience providing treatment and case management services to families.

Accounts Receivable Clerk

This F/T position is responsible for preparing A/R reports, correcting account mispostings including unapplied payments, filing, communicating with collections personnel regarding patient liability, verifying correctness of patient account balances, and ensuring that monthly patient statements are mailed on time which may include doing in-house mailings.
Basic knowledge of Microsoft office packages including a good understanding of Excel, good communication skills, 10-key, detail oriented and good organizational skills are required. Also, a working knowledge of Medical Manager including the collection module is preferred.

Outreach Worker/Case Manager position

Southwest Utah Community Health Center, a non profit primary health care agency, is seeking an independent, highly organized individual to serve as Outreach Worker/Case Manager. Responsibilities include working with medical and mental health staff to support and facilitate communication, services, and follow up care for prenatal and chronic care patients, coordinating well checks and immunizations with parents of children under 2 years of age, assisting patients with keeping appointments including referrals to specialists. Excellent interpersonal skills, effective written and verbal communication and detail oriented individual needed.
