Mission
The mission of Bear Lake Community Health Center, Inc. is to provide access to quality primary and urgent health care for the residents and visitors of the Bear Lake Valley and surrounding communities on an ability-to-pay basis. The center will take a holistic approach to maintaining a healthy community through education, prevention, and a community networking system.

Overview
Bear Lake Community Health Center is Utah's newest CHC, having begun operation in August of 2003. BLCHC took over operation of the Fast Aid Clinic site, formerly owned by Bear Lake Memorial Hospital in Montpelier, ID. The clinic was preparing to close its doors due to lack of income and would have left the residents of Rich County with a lengthy drive to any medical service provider. The successful funding by the Bureau of Primary Health Care will ensure that the residents and visitors to the Bear Lake area will be able to access high-quality medical services.

Services Provided at BLCHC

Pediatric/Adolescent Care--Well child exams, physicals, chronic condition care, immunizations

Adult Care--Employment/CDL/Mission physicals, routine health exams, female GYN exams and care, treatment of chronic conditions such as hypertension, asthma, diabetes, cardiovascular disease

Urgent Care Services--Acute illnesses, laceration and minor injury treatment, non-life threatening medical treatment, X-ray, lab, and pharmacy services, 24-hour medical coverage

Dental Services--Evaluations, exams, and preventive care, restorative care such as fillings, crowns, and extractions, emergency care

Mental Health Services--Treatment and counseling, developmental screening, 24-hour crisis care, substance abuse counseling
