Mission
To provide quality patient and family-centered primary care services to individuals who have inadequate access to health care.
Overview
Community Health Centers, Inc. (CHC), a private
non-profit, 501(c) 3 organization, has been a member of the community health
center movement since 1979 and serves a medically underserved area (MUA) and
health provider shortage areas (HPSA) for primary care, dental and mental health
services. CHC is a safety net provider of primary health care to Salt Lake
County’s uninsured, low-income families. From simple roots as a “free clinic,”
CHC has grown to encompass four Salt Lake County-based health centers, two
dental clinics and a rural farm worker clinic.
The CHC health centers offer a comprehensive range of primary health care,
including: prenatal and delivery, obstetrics/gynecology, immunizations,
individual and family counseling, periodic screening and acute episodic care.
Ancillary services offered by CHC include laboratory testing, radiology, free
and low-cost pharmacy, chronic illness management and patient education. Dental
services include general dentistry, preventive services, dental hygiene and
patient education. CHC also provides limited optometry and have arranged for low
cost ophthalmology services.

Migrant Services
In 1990, Community Health
Centers, Inc. (CHC) received Federal funding to provide health services to
Utah's migrant and seasonal farmworker population. With this funding, CHC
provides medical, dental, health education, and outreach services through Utah
Farm Worker Program (UFWH) at
the Brigham City site,
Clinica de Buena Salud. UFWH
also refers patients to other existing health care providers and resources.
UFWH is based on the primary care model. The program maintains a year-round
clinic one full day a week at the site in Brigham City located in northern
Utah's Box Elder County. UFWH also provides regular dental services two days a
week and one evening a month at the Brigham City site. Additionally, UFWH
supports year-round primary care of farmworkers through contracts with Midtown
Community Health Center in Ogden and Mountainlands Community Health Center
located in Provo. Emergency dental and other specialty care is provided through
contract with community providers.
