Montezuma Creek Health Clini

Utah Navajo Health Systems, Inc. (UNHS) serves San Juan County, Utah. The service area includes the Utah Strip of the Navajo Nation and the un-served and the underserved population of San Juan County. Montezuma UNHS provides medical, behavioral health and dental services for approximately 6000 Navajo and 1000 Anglo people.

The mission of UNHS is to make a difference in the quality of life for all community members by providing high quality, comprehensive primary and preventive health care in a culturally and linguistically competent manner while maintaining fiscal viability.

Unique Characteristics
The population served is truly frontier; 85% of the population has no running water, electricity, telephone or central source of heat; 56% are unemployed; per capita income is $4,106.00; 57% of the population is below the Federal Poverty Level (FPL), 32% below 200% FPL. The population is predominantly Navajo-speaking. Travel time to the clinic facility is one to three hours one way - most of which is on unpaved roads, often rendered impassable because of weather conditions. UNHS serves an area dense with oil and gas wells and its attendant high risk of serious injury. The clinic is the only source of care for Native American patients in a 100 mile service area.

Services
UNHS provides comprehensive primary and preventive medical, dental and mental health services in a culturally appropriate and sensitive manner with a family oriented focus. This includes translation and/or Navajo/English bilingual access for all patients receiving any and all services at the UNHS facilities. Delivery sites include the primary delivery site in Montezuma Creek, Aneth Community School, Bluff Community Center and the Diabetes Control Project Community Education, an Outreach serving the Montezuma Creek and Monument Valley communities. UNHS also includes Blanding Family Practice which provides primary care, full time 5 days/week. A new facility is currently being constructed in Blanding and will include additional provider space, lab, x- ray and patient education areas. UNHS anticipates a significant increase in services and delivery sites in the near future.
