AUCH provides support to members in their efforts to provide quality care to patients with chronic diseases.


Heart Disease and Stroke Prevention

The Utah Department of Health, Heart Disease and Stroke Prevention program partners with AUCH to provide contracts to AUCH members to enhance the quality of care for patients with cardiovascular diseases.  For fiscal year 2010 (July 1, 2009- June 30, 2010) five AUCH members are receiving contracts to support their efforts around cardiovascular disease.  Health centers were chosen for funding based on their applications in response to a Request for Proposal   (RFP) process.  The RFP asked member health centers to choose one area of the Care Model to address and to show that they have a comprehensive quality improvement infrastructure to do this. 

The five health centers that are being funded and their chosen area of the Care Model and disease focus are:

Bear Lake Community Health Center, Cache Valley- Clinical Information Systems for patients with High Cholesterol

Green River Medical Center- Self-Management Support for patients with High Blood pressure

Midtown Community Health Center- Health Care Organization for patients with High Blood Pressure

Mountainlands Community Health Center- Self-Management Support for patients with High Cholesterol

Wayne Community Health Center- Self-Management Support for patients with High Blood Pressure


Diabetes Prevention and Control

The Utah Department of Health, Diabetes Prevention and Control program partners with AUCH to provide contracts to AUCH members to enhance the quality of care for patients with diabetes.  For fiscal year 2010 (April 1, 2010- March 31, 2010) five AUCH members are receiving contracts to support their efforts around cardiovascular disease.  Health centers were chosen for funding based on their applications in response to a Request for Proposal   (RFP) process.  The RFP asked member health centers to choose one area of the Care Model to address and to show that they have a comprehensive quality improvement infrastructure to do this. 

The five health centers that are being funded and their chosen area of the Care Model are:

Carbon Medical Services Association, Inc.- Self-Management Support

Community Health Centers, Inc.- Self-Management Support

Green River Medical Center- Self-Management Support

Mountainlands Community Health Center- Self-Management Support

Southwest Utah Community Health Center (now Family Healthcare)- Self-Management Support
