Basic Information
What is UDS?
The Uniform Data System is “a core set of information appropriate for reviewing the operation and performance of health centers.” (Health Center Program: Uniform Data System, http://bphc.hrsa.gov/uds/)
The UDS collects several different types of information from health centers on an annual basis. The data can be used for health centers to assess themselves in comparison to other health centers that are similar to their patient population and location.
Data Collected includes:
- Financial Information including costs and revenues
- Patient Demographics
- Services Provided
- Staffing
- Clinical Indicators
- Utilization Rates
Grantees Required to Report:
The UDS is a reporting requirement for grantees of the following HRSA primary care programs:

Community Health Center

Migrant Health Center
-
Health Care for the Homeless
-
Public Houseing Primary Care
Use of Data
The data are reviewed to ensure compliance with legislative and regulatory requirements, improve health center performance and operations, and report overall programs and identify effective services and interventions to improve the health of under-served communities and vulnerable populations. UDS data are compared with national data to look at differences between the U.S. population at large and those individuals and families who rely on the health care safety net for primary population at large and those individuals and families who rely on the health care safety net for primary care. UDS data also inform Health Center Program grantees, partners, and communities about Health Centers and their patients. (Health Center Program: Uniform Data System, http://bphc.hrsa.gov/uds/)

Health Centers are also encouraged to set goals for their programs based on UDS data collected so that they can show improvement over time, especially in around the clinical indicators collected.
