DSME Workgroup Monthly Meeting

Overview: Monthly meeting with Diabetes Self Management Education work group

During the six-month initiative, Health Centers will have the chance to earn up to $3,000 and learn how to implement a DSME program or Diabetes Group Visits. AUCH will also host monthly webinars, Care Coordination trainings, and provide additional resources to those who participate.

Definition of DSME/T
Diabetes self-management education and training (DSME/T) is a collaborative process through which people with diabetes gain the knowledge and skills needed to modify their behavior and successfully self-manage the disease and its related conditions. This process incorporates the needs, goals, and life experiences of the person with diabetes and is guided by evidence-based standards.
