Note: Participation is limited to Community Health Centers (CHCs) staff. The Cherokee Model is team oriented, leveraging the patient-centered medical home for enhanced behavioral health care. To maximize the benefits of this training, at least two participants from the CHC should attend, one representing the CHC’s behavioral health services and one representing primary care services.
