Overview: Who are your most vulnerable patient populations? Practices can systematically use patient risk levels to make better care management decisions, such as providing better access and resources to patients in these higher risk levels and more proactively manage their patient population.

AUCH is excited to announce the launch of a Quality Improvement (QI) project focused on identifying who to target, why we are targeting that population, what data to use to determine risk levels when the data should be reviewed, and how to best manage the care for this target population.

Risk stratification is well-documented as central to successful care management as its application enables care teams to predict outcomes, prioritize interventions, and prevent adverse events. This project is an initial step in preparing Health Centers for successful participation in value-based care. A companion effort directs the health system to strengthen their approach to population health management, improving clinical outcomes in preventive and chronic care.

By the end of this initiative, Utah’s Health Centers will:
• Attend four trainings focused on the advancement of current QI efforts through data management, applied risk stratification, and enhanced reporting capabilities. 

• Identify and implement a structured, algorithmic approach to determine patients' risk levels to drive better care management and team support.

• Receive one-on-one consultation to support identifying patient risk levels and integrating these risk levels into the management of chronic care patients.

• Participate in monthly QI and HIT peer groups to identify challenges and share promising practices related to this initiative.

Presenter Bio: AUCH is partnering with Ms. Bonni Brownlee, MHA, CPHQ, PCMH-CCE to provide trainings and consultation on the project. For the past 14 years, Bonnie focused her attention on clinical practice transformation, providing expertise in practice redesign and evolution towards the Patient-Centered Medical Home (PCMH) model of care to countless primary care associations, clinical practices, health systems, and health plans.
