End of Life Care: Sharing Your Wants and Wishes

HealthInsight is hosting a collaborative community event to promote end of life care awareness and the importance of having planning conversations. Patients, families, researchers, providers, policymakers, payers and the local community are invited to this event to participate in interactive presentations and discussions. We will address end of life care issues in Utah and encourage community member input on planning conversations, policy change, and fostering alliances between patients and providers.

At the end of this conference, participants will:

  • Understand why advance care planning is important
  • Understand how to share advance care planning decisions with family and health care providers
  • Be able to effectively address advance care planning communication barriers in your family
  • Be able to find the appropriate tools, resources, and contacts for you and your family's advance care planning needs
